Job Overview:
We are seeking a skilled and compassionate Licensed Vocational Nurse to join our healthcare team. As a Licensed Vocational Nurse, you will provide direct patient care under the supervision of a registered nurse or physician. Your primary responsibilities will include administering medications, monitoring vital signs, and assisting with patient assessments. Responsibilities:
- Administer medications and treatments as prescribed by healthcare providers
- Monitor and record vital signs, such as blood pressure, temperature, and pulse
- Assist with patient assessments and document findings
- Perform phlebotomy procedures and collect laboratory specimens
- Provide wound care and suture removal as needed
- Assist with tube feedings and ensure proper nutrition for patients
- Collaborate with interdisciplinary healthcare team members to develop and implement patient care plans
- Educate patients and their families on healthcare procedures, medication administration, and self-care techniques
- Provide primary care services to individuals with developmental disabilities or behavioral health needs
- Maintain accurate and up-to-date medical records, route sheets and notes on HospiceMD.
Qualifications:
- Valid Licensed Vocational Nurse (LVN) license in the state of [STATE]
- Previous experience in acute care or primary care settings preferred
- Proficient in phlebotomy procedures and laboratory specimen collection
- Familiarity with working with individuals with developmental disabilities or behavioral health needs
- Ability to perform tube feedings and provide wound care/suture removal
- Experience working with children is a plus
- Strong communication skills and ability to work well within a team
- Excellent attention to detail and organizational skills
